  • Date and time booking received
  • SNT employee processing the booking
  • Account holder
    • Order reference #
    • Cost Code / Dept / Ward etc
  • Client Emloyee making the Booking
  • Name of Passenger
  • Mobility status of passenger i.e. Elect - Manual Wheelchair /With Escort / Ambulatory / Walking Frame etc plus any speciific special instructions i.e vision impaired
  • Type of vehicle used
  • Pick-up and Destination Address's (including Lat - Long co-ordinates)
  • Distance traveled
  • Date / Time the job is allocated to the driver
  • Time Driver arrives at Pick-up address
  • Time passenger on board and driver departs Pick-uo address
  • Time job is completed

All the above data can be provided in Spreadsheet format for sorting or enterig into another database.
